朕は、人口動態調査令を改正する勅令を裁可し、ここにこれを公布せしめる。
I hereby give My Sanction to the Imperial Ordinance concerning the amendment to be made to the Ordinance regarding Vital Statistics and cause the same to be promulgated.

人口動態調査令
Imperial Ordinance regarding Vital Statistics
第一條　人口の動態を調査するため必要な資料は、この勅令の定めるところにより、これを徵集する。
Article 1.　Materials necessary for compilation of vital statistics shall be collected in accordance with the provisions of the present Imperial Ordinance.
第二條　人口動態調査資料は、出生、死亡、死產、婚姻及び離婚につき、その屆出を受けた市町村長が、人口動態調査票及び人口動態統計月報により、これを提出しなければならない。人口動態調査票は、出生票、死亡票、死產票、結婚票及び離婚票の五種とする。
Article 2.　Materials for compilation of vital statistics regarding births, deaths, stillbirths, marriages, and divorces shall be presented in the form of Vital Statistics Schedules and Monthly Reports of Vital Statistics by the mayor or the headman of town or villlge who has received reports of such occurrences. Vital Statistics Schedules shall consist of five kinds, i.e., Birth Schedules, Death Schedules, Stillbirth Schedules, Marriage Schedules, and Divorce Schedules.
棄兒發見による就籍は、出生に準じ、就籍地の市町村長か、失踪及び死亡確認による除籍は、死亡に準じ、本籍地の市町村長が、夫々人口動態調査票を作成しなければならない。
Regarding the registration of a discovered founding in a census register, the Vital Statistics Schedule shall be drawn up, in the same way as in the case of a birth, by the mayor of the city or the headman of the town or village at whose local office the foundling is to be legally registered. Regarding the removal from a census register of the name of a person due to disappearance or the confirmation of death, the Vital Statistics Schedule shall be drawn up, in the same way as in the case of a death, by the mayor of the city or the headman of the town or village where the person's Permanent Domicile is situated.
第三條　市町村長は、戶籍法による屆書又は昭和二十一年厚生省令第四十二號による屆書その他の關係書類に基いて、內閣總理大臣の定めるところにより、人口動態調査票及び人口動態統計月報を作成しなければならない。
Article 3.　The mayor or the headman of town or village shall draw up Monthly Reports of Vital Statistics and Vital Statistics Schedules, as provided by the Prime Minister's Ordinance, on the basis of reports made in accordance with the Census Registration Law, reports made in accordance with Ministry of Welfare Ordinance No. 42 of 1946 and other papers concerned.
第四條　人口動態調査票及び人口動態統計月報の用紙は、これを市町村長に交付する。
Article 4.　The forms for the Vital Statistics Schedule and the Monthly Reports of Vital Statistics shall be sent to the mayor or the headman of town or village.
第五條　人口動態調査票及び人口動態統計月報は、これを每月、市町村長は府縣知事に、府縣知事は內閣總理大臣に提出しなければならない。
Article 5.　The Vital Statistics Schedules and the Monthly Reports of Vital Statistics shall be sent monthly to the prefectural governor by the mayor or the headman of town or village; and then to the Prime Minister by the Prefectural Governor.
第六條　この勅令では、市町村長には、東京都、京都市、大阪市、橫濱市、名古屋市及び神戶市の區の區長を、府縣知事には、東京都長官及び北海道廳長官を含む。
Article 6.　In the present Ordinance, the "mayor or the headman of town or village" include the "head men of wards" in Tokyo Metropolis, Kyoto City, Osaka City, Yokohama City, Nagoya City, and Kobe City; and "the Prefectural Governors" include the "Governor-General of Tokyo Metropolis" and "Governor-General of Hokkaido."
附　則
Supplementary Provisions:
この勅令は、昭和二十一年十月一日から、これを施行する。
The present Ordinance shall come into force as from October 1, 1946.
昭和二十年勅令第五百號人口動態調査令臨時特例は、これを廢止する。
Imperial Ordinance No. 500 of 1945, "Provisional Exceptional Regulations regarding Vital Statistics" is hereby abolished.
