      <Article Num="1">
        <ArticleTitle>Article 1.</ArticleTitle>
        <Paragraph Num="1">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence>Materials necessary for compilation of vital statistics shall be collected in accordance with the provisions of the present Imperial Ordinance.</Sentence>
          </ParagraphSentence>
        </Paragraph>
      </Article>
      <Article Num="2">
        <ArticleTitle>Article 2.</ArticleTitle>
        <Paragraph Num="1">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence Num="1">Materials for compilation of vital statistics regarding births, deaths, stillbirths, marriages, and divorces shall be presented in the form of Vital Statistics Schedules and Monthly Reports of Vital Statistics by the mayor or the headman of town or villlge who has received reports of such occurrences. Vital Statistics Schedules shall consist of five kinds, i.e., Birth Schedules, Death Schedules, Stillbirth Schedules, Marriage Schedules, and Divorce Schedules.</Sentence>

        </Paragraph>
        <Paragraph Num="2">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence>Regarding the registration of a discovered founding in a census register, the Vital Statistics Schedule shall be drawn up, in the same way as in the case of a birth, by the mayor of the city or the headman of the town or village at whose local office the foundling is to be legally registered. Regarding the removal from a census register of the name of a person due to disappearance or the confirmation of death, the Vital Statistics Schedule shall be drawn up, in the same way as in the case of a death, by the mayor of the city or the headman of the town or village where the person's Permanent Domicile is situated.</Sentence>
          </ParagraphSentence>
        </Paragraph>
      </Article>
      <Article Num="3">
        <ArticleTitle>Article 3.</ArticleTitle>
        <Paragraph Num="1">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence>The mayor or the headman of town or village shall draw up Monthly Reports of Vital Statistics and Vital Statistics Schedules, as provided by the Prime Minister's Ordinance, on the basis of reports made in accordance with the Census Registration Law, reports made in accordance with Ministry of Welfare Ordinance No. 42 of 1946 and other papers concerned.</Sentence>
          </ParagraphSentence>
        </Paragraph>
      </Article>
      <Article Num="4">
        <ArticleTitle>Article 4.</ArticleTitle>
        <Paragraph Num="1">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence>The forms for the Vital Statistics Schedule and the Monthly Reports of Vital Statistics shall be sent to the mayor or the headman of town or village.</Sentence>
          </ParagraphSentence>
        </Paragraph>
      </Article>
      <Article Num="5">
        <ArticleTitle>Article 5.</ArticleTitle>
        <Paragraph Num="1">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence>The Vital Statistics Schedules and the Monthly Reports of Vital Statistics shall be sent monthly to the prefectural governor by the mayor or the headman of town or village; and then to the Prime Minister by the Prefectural Governor.</Sentence>
          </ParagraphSentence>
        </Paragraph>
      </Article>
      <Article Num="6">
        <ArticleTitle>Article 6.</ArticleTitle>
        <Paragraph Num="1">
          <ParagraphNum/>
          <ParagraphSentence>
            <Sentence>In the present Ordinance, the "mayor or the headman of town or village" include the "head men of wards" in Tokyo Metropolis, Kyoto City, Osaka City, Yokohama City, Nagoya City, and Kobe City; and "the Prefectural Governors" include the "Governor-General of Tokyo Metropolis" and "Governor-General of Hokkaido."</Sentence>
          </ParagraphSentence>
        </Paragraph>
      </Article>

    <SupplProvision>
      <SupplProvisionLabel>Supplementary Provisions:</SupplProvisionLabel>
      <Paragraph Num="1">
        <ParagraphNum/>
        <ParagraphSentence>
          <Sentence>The present Ordinance shall come into force as from October 1, 1946.</Sentence>
        </ParagraphSentence>
      </Paragraph>
      <Paragraph Num="2">
        <ParagraphNum/>
        <ParagraphSentence>
          <Sentence>Imperial Ordinance No. 500 of 1945, "Provisional Exceptional Regulations regarding Vital Statistics" is hereby abolished.</Sentence>
